In case where head office of importation / manufacturing premise is located in Bangkok Metropolitan area, application form shall be filed to Food Control Division, Food and Drug Administration. Except the application form of Temporary Import Food into the Kingdom License.

In case where head office of importation / manufacturing premise is located in provincial area, application form can also be filed to that particular provincial public health offices. Except the application form of Temporary Import Food into the Kingdom License. And, if, such foods are empowered to the provinces to control, the provinces are the authorities to approve the application.

The application form of Temporary Import Food into the Kingdom License shall be filed to Import-Export Inspection Division, Food and Drug Adminsitration.

- Every application forms have to submit at One Stop Service Center
- There are eighteen licenses can get approved in one day as follow :

2. apply for Amending Food record/Food declaration	60 items
3. apply for Food Manufacturing Replacement License	15 items
4. apply for Food Import Replacement License
5. apply for Food Registration Replacement
6. apply for Food Production License Copy	10 items
7. apply for Amending Food Production License Copy
8. apply for Food Labelling Copy
9. apply for Amending Food Labelling Copy
10. apply for Food record/Food declaration Copy
11. apply for Amending Food record/Food declaration Copy
12. apply for Renew Food Manufacturing License (have been certified GMP already)	25 items
13. apply for Renew Food Import License	25 items
14. apply for Amending Food Manufacturing License *	5 items
15. apply for Amending Food Import License *	5 items
16. apply for Amending Food Production License *	5 items
17. apply for Amending Food Registration *	5 items
18. apply for Amending Food Labelling *	5 items

      * note = only amending details as follow :
                          - Amending Company’s or Licensee’s name
                          - Amending name and surname of Holder
                          - Amending name and surname of Superintendent
                          - Amending address of Licensee’s Premise
                          - Amending Food Categories Approval
