Documents required for application of Manufacturing License and Production License

 

 Applicant who intends to establish a food plant shall submit petition together with the following documents.

1. An application form of Food Manufacturing License of Food Production License (Form Orr.1, Form Sor Bor.1) filed at Food Control Division FDA or at Public Health Office in the provincial areas.

2. A copy of domestic registration. If an applicant is alien, a work permit in Thailand which issued by the Labour Department shall be attached.

3. A copy of Trade or Commercial Registration

4. A copy of company registration which declared objectives and authorised person of the Company.

5. A copy of certifying nationality of company. (List of share holders) from the Ministry of Commerce. If an applicant is alien, a certificate of operating business in Thailand is required.

6. Certificate of proxy to action behave of applicant entity (for juristic person) with 30 Bath excised tax. The company stamp is also required if specified in the company registration.

7. 2 sets of the following scaled plans.

     7.1 Plant layout of the production building

     7.2 plan indicating other building or compound of proposed plant, including waste water treatment and artesian well. (if any)

     7.3 Plans indicating the following details.

           (1) Front view, side view and cross section of the production building.

           (2) Floor plan indicating

                 - location of all equipments and machines, including the detail of size, capacity, horse power, etc. If the horse power is not indicated, ampare, or voltage or watts of each electrical equipment should be specified. Each boiler shall be indicated either its boiler horse power as shown-on a name plate and its steam rate used or its design.

                 - The level of production floor, which shall be sloped so as to facilitate cleaning.

                 - Full detail of drainage system with the size and direction from inside to outside of the plant.

           (3) full detail of water treatment system.

           (4) Partitioning areas shall be provided for different purposes.

                 - raw material, packing material and finished products.

                 - preparation of raw material

                 - processing

                 - packing

                 - labelling

                 - cleaning all utensils and equipments.

                 - others

           (5) Location and the number of men's and women's toilet hand washing facilities and men's urinals.

     7.4 2 sets of full details in the production lines.

           (1) source of water used in processing, method treating water also be attached.

           (2) full details of processing procedures of each food products.

           (3) methods of washing or cleaning of all utensil and equipment (if any).

           (4) types of food produced in the plant, contains and packing material used and the yield of products per day or per month

           (5) types of raw materials and ingredients.

           (6) method of waste disposal treatment.

           (7) the number of workers, male and female.
